The medical report is very specific information regarding the patient in which any healthcare provider can look at and quickly assess the status of the patient. The patients health care notes can be written ahead of time by the doctor before there requested or after third-party payer request medical information for payment. Two of the more common reasons doctors do not get paid in time by third-party carriers is they do not establish the need for care or they fail to support the need for care with the personal injury patient notes in a timely manner.
Chiropractors are often late in delivering medical reports primarily because they're not familiar with how easy they can be to create. The best system for giving a medical report is using voice dictation combined with a template of the topics to be included in the narrative report. And so it's very easy for the doctor to look at the patient's notes, look at the template see what information should be gathered, and then voice dictate the information directly into a document. It only takes about two or three days of using the template combined with the patient's records and voice dictation to get very comfortable using the process. There are plenty of voice dictation software systems now available. It can be as easy as using a digital voice recorder with a template and talk directly into the recorder. And then you can take the recording within the voice recorder plugged into a software program such as Dragon and it will transcribe your voice into the patients report. There are several categories that should be included within each of the patients health records.
Some of the major categories to include in the medical reports or the brief history of the chief complaint, the present subjective findings, the present examination and diagnostic findings, the therapeutic protocol, the patient's response to treatment and the short term goals for the patient. Many of the sections are common phrases that are routinely used however personalized to the patient's findings. And that's why it gets very comfortable and easy to voice dictate medical reports after two or three days. In addition the template forms follow the same subject matter which also makes it easier to remember. The process is as easy as looking at which part of the template neuron and then turning to the medical records and vocalizing that category of Mac medical records into the voice recorder. And then repeat for the very next section.
Some of the electronic health software has the ability for voice dictation that is transcribed into the notes. In this type of electronic software makes it very fast and easy to record the patients health care status. Using dictation in a voice recorder to create the medical reports also allows for high specificity and much more accurate information to be documented in a very short period of time. As an example a typical medical report when using the template associated with the patient's records takes as little as 3 to 5 minutes. The patients status should be produced upon the initial examination and after every re-examination. They are mostly used for third-party reimbursement. However the patients health care records are the single most common way for healthcare professionals in the medical legal community to communicate the patient's status. And if the doctor gets used to briefly creating a health care report after the examination re-examinations for every type of patient it significantly helps communicating with all healthcare providers attorneys and third-party re-inverse providers.
It is common professional procedure to automatically create a medical report when patients have been referred to you by other healthcare providers or personal injury attorneys. It's a good way to communicate with your referring community without them having to take time out to ask you how the patient is proceeding.
